Step One.

Step Two.
Print the template PDF file onto a sheet of A4 paper.

Step Three.
Fold the sheet of paper in half along the dotted line.

Step Four.
Fold the two corners into the center along the dotted lines.

Step Five.
Fold the two remaining flaps over twice to form a rim.
